ABOUT THE ROLE
Under the direct supervision of the Clinical Manager, and working as a member of the North West Mental Health & Addictions Hub, the Access Point Facilitator is responsible for the triage, screening & service matching of MHA referrals to programs and services. This includes verifying information as needed with clients and referral sources and entering information into the electronic database. The Access Point Facilitator is also responsible for conducting screening appointments with all referred clients for Mental Health & Addictions programming. Superior customer service for both client and referral source is most important in performing this job. The Access Point Facilitator will work closely with other Access Point Facilitators and members of the interprofessional teams within the catchment of the North West Provincial Coordinated Access.

WHAT YOU BRING
Education/Experience:
- Degree or diploma in the Health and Human Services Field
- Knowledge and experience with referral processes, triage, and system navigation, assessing and treating individuals with serious and persistent mental illness, and/or addictions
- Demonstrated experience with technology, documentation in electronic medical records and working with electronic databases
- Minimum three years’ experience in mental health and addictions providing direct client care or care coordination
Skills/Abilities:
- Maintains an awareness of ethical guidelines and practices for the provision of effective client centred care
- Knowledge of relevant legislation, e.g., Mental Health Act, Personal Health Information Protection Act, Regulated Health Professions Act, Health Care Consent Act, and professional standards to ensure compliance and consistency in provision of client care
- Demonstrated excellent interpersonal communication skills (oral and written) to effectively communicate with clients, referents and community agencies
- Demonstrated experience with technology, documentation in the Electronic Medical Record and working with electronic databases
- Time management and organizational skills in order to effectively address and prioritize multiple tasks and timelines in a fast-paced environment
- Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving and time management skills
- Demonstrated bio-psycho-social-spiritual assessment skills
- Ability to work closely with others including other referral facilitators and inter-professional teams
- Knowledge of a range of theoretical perspectives, psychosocial rehabilitation principles, chronic disease self-management, and holistic wellness approaches
- Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving and time management skills
- Knowledge and understanding of relevant legislation (i.e., Mental Health Act, Health Care Consent Act, PHIPA)
- Demonstrated regular attendance at work
Asset:
- Registration with the appropriate professional regulatory college (e.g. Ontario College of Social Workers and Social Service Workers)
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
